SCOPE:

� Provides client education on health habits, self-examinations, behavior management of

other concerns relating to medical and health issues.

� Establishes patient care goals and reinforces understanding of disease processes and illness management, medication and related issues, medical resources, and self care skills

to clients.

� Maintains continuity of care by documenting and communicating care and on-going plans

of treatment.

� Researches, develops, implements, trains and promotes company wellness programs, assisting in education of patients and the target community in preventive health care and health maintenance concepts.

ESSENTIAL JOB FUNCTIONS:

COMMUNICATION Communicates with attending physician, Hospice Medical Director, members of the IDT, and other agencies as needed to coordinate optimal care and use of resources for assigned patients/families. Maintains regular communication with the Patient Care Coordinator and Director of Clinical Services.

NEEDS ASSESSMENT Receives patient assignments from the PCC or DOCS. Assumes case management responsibilities for assignment including assessing, planning, implementing, reevaluating, and revising care as necessary. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.

CASE MANAGEMENT Coordinates all services provided to patients/families in all settings to ensure quality of care/services as well as appropriateness of care.. Supervises implementation of the approved plan of care.

PATIENT RECORDS Maintains current patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur. Initiates and sustains the implementation of orders for medications and treatments as prescribed by the physician in the plan of care. Completes electronic point of care charting on patients/families. Participates in clinical audits as assigned by the PCC or DOCS and completes tasks requested to remain in compliance with federal and state regulations.

COORDINATING PATIENT CARE Accepts responsibility for managing overall care of the patient by providing direct patient care, and by coordinating the care of the patient by teaching primary caregivers, volunteers, and employed caregivers as appropriate. Attends all team specific IDT meetings presenting his/her patients and making changes in the POC as agreed to by the IDT. Attends care conferences as required by LTC regulatory policy. POLICY AND PROCEDURE Demonstrates familiarity with policies of the hospice and state and federal regulations pertaining to nursing services. Responsible for adhering to conditions of participation as outlined by regulatory agencies.

CODE OF CONDUCT AND BUSINESS ETHICS Must adhere to Code of Conduct and Business Ethics policy including the documentation and reporting responsibilities.

QAPI Participates in the Hospice�s Quality Assessment and Performance Improvement Program and Hospice sponsored in-service training.

CONTINUING EDUCATION Hospice personnel are expected to participate in orientation, ongoing educational programs and appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs. Maintains required contact hours/continuing education units as required for licensure/certification.

OTHER Participates on committees and special projects and performs other duties as assigned. Accepts responsibility of assignments to perform specialized procedures, such as IV therapy, only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.

RESPONSIBILITIES: Responsible for assessment, planning, coordination, implementation and evaluation of injured/disabled individuals involved in the medical case management process. Working as an intermediary between carriers, attorneys, medical care providers, employers and employees, you will closely monitor the progress of the injured worker and report results back to the employer and insurance carrier. This will ensure appropriate and cost-effective healthcare services leading to a medically rehabilitated individual who is ready to return to an optimal level of work and functioning.

REQUIREMENTS: Diploma, A.S. degree or B.S. degree in nursing required. 2+ years clinical nursing experience required. Case Management and/or Workers� compensation-related experience strongly preferred. Registered nurse with current, valid state licensure required. Must be eligible to sit for CCM certification and must have a valid driver�s license. Excellent interpersonal skills, phone manners and organizational skills are essential. Must be proficient in Microsoft Office suite with such programs as Word and Excel as well as have experience with internet searches and a comfort level with other software programs. Ability to set priorities and work independently is essential. Spanish speaking ability a plus!
